Thank you for considering using our online wellness practice. Please read these terms and conditions carefully before using Rapid Rehab operated by Rapid Rehabilitation LLC

By accessing or using the Rapid Rehab, you agree to be bound by these Terms. If you disagree with any part of the terms, then you may not access the Rapid Rehab.

 

Online Wellness Practice

Our online wellness practice offers services and products designed to improve the mental, emotional, and physical well-being of our clients. These services explicitly do not include physical therapy or any other services related to the treatment of physical injuries or ailments. Services are rendered by licensed Doctors of Physical Therapy, but services are provided from wellness consultation aspect of rehabilitation.

Medical Screening and Consent Forms

Our intake forms are provided in order to better serve our clients in the most efficient way. Therefore, consent form and medical screening must be completed prior to consultation start time. Clients that have not submitted it must do so at the beginning of their appointment and appointment times will not be extended.

Eligibility

In order to use our online wellness practice, you must be at least 18 years old. By using our services, you represent and warrant that you meet this age requirement and have the legal capacity to enter into a contract. Users under 18 years of age are able to qualify for services if authorized by signature of parent/legal guardian.

HIPAA Compliance and Data Protection

Ensuring the privacy and security of patient information is of utmost importance to Rapid Rehabilitation LLC. Our online wellness practice adheres to the Health Insurance Portability and Accountability Act (HIPAA) to safeguard the confidentiality and integrity of your personal health information.

In order to access certain features of Rapid Rehab, you may be required to create an account. You are responsible for maintaining the confidentiality of your account and password, and for restricting access to your device. You agree to accept responsibility for all activities that occur under your account or password.
